What you will be responsible for
Be responsible for the initial assessment and review of patients, across a broad spectrum of clinical conditions, considering the holistic needs of patients, under the direction of the senior nurse.
Provide clinical care, treatment, advice and interventions, including the transcribing and prescribing of medication if appropriate, through holistic assessment of patient need; and then planning, implementing and evaluating a person centred care plan as appropriate.
Be responsible for admitting and discharging patients from the caseload.
Undertake continuing health care checklist and fast track applications of on-going nursing need and where eligible commission packages of care from external agencies.
The role will involve, but not limited to you performing the above.
You will have the following qualification/experience:
A minimum of 6 months paid experience as a Community Nurse working for a UK hospital within the last 3 years
Comply with NMC Professional Registration and keep up to date with current nursing practices
Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
Undertake regular mandatory training*
Full UK driving licence and own vehicle.
*Depending on the Trust, you may require variable additional certification and skills.
